Instructions for Filing a Claim
There are three different ways for community members to file a claim for expenses incurred as a result of the BioLab fire.
We recognize that the high volume of interest by community members seeking to file claims through the in-person Community Assistance Center has resulted in limited appointment availability. Therefore, we strongly encourage community members to email BioLab’s dedicated claims email (details below) for immediate assistance with filing and processing claims.
Establishing a claim through BioLab’s dedicated claims email
- Email the following information to [email protected]:
- Name
- Address
- Phone number
- Items being claimed/amount of each claimed item
- Attach copies of receipts or other supporting documentation
- You will be contacted by an adjuster by phone to further discuss your claim further.
NOTE: Community members who file and process their claim by email can be assisted immediately without contacting BioLab’s call center or visiting the Center.
Establishing a claim through BioLab’s Call Center
- Call BioLab’s 24/7 hotline at (678) 301-2359.
- Establish your claim with the claims line staff.
- Obtain your claim reference number.
- You will be contacted by an adjuster to review and discuss your claim further.
NOTE: Community members who file and process their claim by phone can be assisted immediately without visiting the Center.
Establishing a claim through BioLab’s in-person Community Assistance Center
- Call BioLab’s 24/7 hotline at (678) 301-2359.
- Establish your claim with the claims line staff.
- Schedule an appointment online by clicking here.
- Gather required documents to bring to the Center:
- A copy of appointment confirmation email (printout or mobile)
- Government-issued ID
- Any relevant documentation and receipts
- Attend your appointment at the Community Assistance Center on the scheduled date and time.
Additionally, if you are a business owner making a claim for loss of business profits, please also gather the following to help us document your claim:
- 2023 tax returns
- 3-6 months of profit and loss statements
- For losses related to contract jobs, a copy of the executed contract with job pricing and proof of cancellation
